I authorize The Ultimate Bride to order a standard size according to the mfg. size chart. I understand that I may require alterations for the gown to fit my specific measurements. An extra charge will apply for some plus size dresses. And an additional fee will apply for (delete) extra length if the bridesmaid is 5'-10" or taller or if extra length is requested. We highly recommend having your measurements taken by a professional seamstress before filling out this form. You will receive your receipt with your dress. An incoming shipping and handling charge per dress will be applied to your account. Standard dress delivery is 3 to 5 months. Your dress will be ordered with the rest of the Bridal party to assure a consistent dye lot(DELETE). No cancellations, all sales are final.
